Nurse Practitioner/Physician Assistant

As a Nurse Practitioner/Physician Assistant, you'll be part of a dynamic team that's making a real difference in people's lives.
Primary Responsibilities:- Deliver High-Quality Care: Provide cost-effective, quality care to assigned members, managing both medical and behavioral, chronic and acute conditions effectively.
- Care Coordination: Facilitate agreement and implementation of the member's plan of care by engaging facility staff, families/responsible parties, primary and specialty care physicians.
- Travel and Flexibility: Travel between care sites is mandatory, and you'll need to be available to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours.
- Certified Nurse Practitioner or Physician Assistant: Must be certified through a national board and have a graduate degree in Nursing (MSN) or Physician Assistant degree program.
- Active and Unrestricted License: Must have an active and unrestricted license in the state where you reside.
- Prescriptive Authority: Must have current active DEA licensure/prescriptive authority or be able to obtain post-hire, per state regulations.
- 1+ Years of Experience: Preferably 1+ years of hands-on post-grad experience within Long Term Care.
- Geriatrics and Chronic Illness: Understanding of Geriatrics and Chronic Illness is a plus.
