Key Responsibilities:

  1. Provide comprehensive evaluation and management of both acute and chronic health conditions.
  2. Order, interpret, and communicate results of diagnostic tests.
  3. Conduct thorough physical examinations and wellness visits.
  4. Follow up with patients post-hospitalization or emergency department visits.
  5. Engage in Transitional Care Management.
  6. Collaborate with a network of physicians and medical professionals to optimize patient care.
  7. Facilitate referrals to specialists and community resources as needed.
  8. Participate in office-based procedures.
  9. Educate and counsel patients to promote physical and emotional well-being.
  10. Support the educational needs of various learners aligned with the goals of the teaching institution.

This role offers the opportunity to develop a patient panel supported by a collaborative team environment.

Qualifications:

The ideal candidate will possess strong communication and teamwork skills, demonstrating the ability to exercise independent judgment and initiative. Completion of NYS NP/PA certification and licensure is required, along with DEA registration prior to the start date.

Compensation:

The salary range for this position is between $93,000 and $119,642. Individual compensation will be determined based on various factors including market data, education, experience, and qualifications.
