Job Summary:
We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our home health care team. The successful candidate will provide high-quality patient care, working independently within the confines of physician orders and as assigned and supervised by a registered nurse.
Responsibilities:
- Provide skilled nursing care to patients in their homes, focusing on promoting health, preventing illness, and alleviating suffering.
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
- Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
- Document patient progress, communicating effectively with healthcare providers and families.
Requirements:
- Graduate of an accredited licensed vocational nursing program.
- Current and valid LVN license in California.
- Proof of car insurance for automobile used for work.
- California Driver License (CA DL).
Preferred Requirements:
- State-licensed LVN with 1 year of nursing experience.
- Med/Surg or acute hospital experience.
- Home health experience.
