Marian Regional Medical Center, a 191-bed facility located in Santa Maria, California, is recognized for its exceptional patient care and commitment to excellence. As a Licensed Vocational Nurse (LVN) in our Home Health department, you will play a vital role in delivering compassionate care to patients in their homes.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current on best practices and evidence-based care.
Requirements
  • Graduate of an accredited LVN program
  • Current and valid LVN license in California
  • Proof of car insurance and a valid California driver's license
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
Preferred Qualifications
  • 1 year of experience as an LVN in a home health or acute care setting
  • Med/Surg or acute hospital experience
  • Home health experience
