About the Role:

This is a full-time position with a competitive salary range of $60,000-$80,000 per year, depending on experience. The ideal candidate will have a strong passion for delivering exceptional patient care and a commitment to excellence in home healthcare.

Responsibilities:
  • Implement and manage patient care plans in collaboration with physicians and other healthcare professionals.
  • Conduct thorough assessments and coordinate care with other healthcare providers as needed.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Document all patient interactions and care provided in accordance with company policies and procedures.
Qualifications:
  • Current, active licensure as a LPN/LVN in the state where the clinician will practice.
  • CPR certification from American Red Cross or American Heart Association.
  • TB skin test (current within last 12 months).
  • At least six months of prior hands-on nursing experience preferred.
