Your primary responsibility will be to serve as a Life Success Specialist, working one-on-one with youth and caregivers to enhance independent living skills and promote economic and social self-sufficiency.
Key Responsibilities- Develop and implement individualized transition plans, goals, and timelines in collaboration with youth, caregivers, and caseworkers.
- Provide direct support training, including group and individual instruction, and hands-on training as needed.
- Work with youth ages 5-17, demonstrating the ability to work with males and females.
- Meet encounter goals each week and maintain regular contact with assigned clients.
- Collaborate with the treatment team to monitor progress and success of interventions, reporting successes and failures as necessary.
- Participate in regular clinical supervision sessions and complete progress notes and necessary paperwork on a weekly basis.
- Attend provider and special parent meetings as required, maintaining confidentiality and professionalism at all times.
- Bachelor's degree in behavioral health or a related field from an accredited university (BHT status preferred).
- Minimum 1-year experience working in the behavioral health field with youth and families.
- Ability to work in a community-based setting, meeting with clients during afternoons, evenings, and weekends.
- Valid Arizona driver's license and insurable under the agency's automobile policy.
- CPR & First Aid Certification, Negative Skin TB Tests or Negative Blood TB Test.
- Computer literacy with knowledge of Outlook and Microsoft Office programs.
