LPN - Skilled Nursing Position

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Eddy Heritage House Nursing and Rehabilitation Center in Troy, NY.

About the Role

As an LPN, you will play a vital role in providing high-quality care to our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, administer medications, and provide emotional support to our residents and their families.

Responsibilities
  • Administer medications and monitor IV therapy as ordered
  • Collect data about residents and determine priorities in resident care based on identified needs and goals
  • Contribute to the plan of care through collaboration and effective communication with the RN and other members of the healthcare team
  • Provide emotional support to residents and their families
Requirements
  • A current license to practice as a Licensed Practical Nurse (LPN) in the State of New York
  • 1 year of experience or more as a LPN, but willing to help train the right candidate to further their skills
  • Ability to maintain the highest level of customer service while completing day-to-day duties and responsibilities
  • Ability to communicate effectively in a fast-paced work environment
