Job Summary

We are seeking an experienced Exercise Physiologist to join our team at Elevance Health. As an Exercise Physiologist, you will play a critical role in providing exercise guidance to disease management members, collaborating with nurse case managers to manage health goals, and implementing primary care physicians' exercise recommendations.

Key Responsibilities
  • Collaborate with nurse case managers to develop and implement personalized exercise plans for disease management members.
  • Implement primary care physicians' exercise recommendations, providing safe and effective exercise instruction and guidance.
  • Provide member health coaching, education, assessment, and support to promote self-management of chronic conditions.
  • Develop and provide education materials related to specific member conditions.
Requirements
  • Bachelor's degree in Exercise Science, Exercise Physiology, Kinesiology, Physical Therapy, or Physical Education.
  • Minimum of 3 years of experience as an Exercise Specialist, Exercise Physiologist, or Fitness Instructor/Trainer developing exercise prescriptions for special populations.
Preferred Qualifications
  • Minimum of 1 year of experience in cardiac rehab, pulmonary rehab, and physical therapy in a clinical setting.
  • Certification by the American College of Sports Medicine (ACSM) as a Health Fitness Instructor.
  • Master's degree in a related field and membership in the American Society of Exercise Physiologists.
