Molina Healthcare is seeking a highly skilled Family Nurse Practitioner to join our team in providing high-quality primary care services to patients in their homes, communities, and nursing facilities. As a key member of our care team, you will be responsible for delivering comprehensive medical assessments, developing treatment plans, and coordinating care with specialists as needed.
Key Responsibilities- Provide general medical care and care coordination to patients of all ages, including adults, women's health, pediatric, and geriatric populations
- Conduct comprehensive evaluations, including history and physical exams, to identify gaps in care and prevent complications
- Address chronic and acute primary care complaints, and determine medical urgency
- Seek specialty consultation as appropriate, and order diagnostic testing to inform medical diagnoses
- Develop and implement medical plans of care, and schedule patient appointments as needed
- Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization
- Participate in community-based 'Pop Up Clinics' to build relationships with community members and address gaps in healthcare
- Drive up to 120 miles per day to various locations within the assigned region, and may be required to walk flights of stairs while carrying equipment
- Master's degree in family health from an accredited nursing program
- Advanced computer skills, including proficiency with Word, Excel, and Electronic Medical Record systems
- Active and unrestricted national certification from a recognized organization, such as the American Academy of Nurse Practitioners or American Nurses Credentialing Center
- Current state-issued license to practice as a Family Nurse Practitioner, and Basic Life Support for Healthcare Professional certification
- 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, ideally in a home health, community health, or public health setting
- Previous experience in home health as a licensed clinician, especially in management of chronic conditions
- Fluency in a language in addition to English is a plus
