Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Case Manager. In this role, you will provide direct care and coordinate with our interdisciplinary team to ensure that patients and their families receive optimal care and support.

Responsibilities
  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborate with our healthcare team to ensure seamless care coordination and communication.
  • Administer medication and treatment as prescribed by the physician, and provide education to patients and families on their care plans.
  • Provide emotional support and counseling to patients and families, and facilitate their involvement in the care planning process.
  • Maintain accurate and detailed clinical notes, and participate in interdisciplinary team meetings to discuss patient progress and care plans.
  • Ensure compliance with all state and federal regulations, and maintain up-to-date knowledge of hospice care best practices.
Requirements
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation.
  • At least one year of nursing experience, with a preference for experience in hospice or a similar setting.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
