Staff - Float Physical Therapist - Healthcare Professional

Advocate Aurora Health is seeking a skilled Physical Therapist to join our team in Oak Lawn, Illinois. As a Float Physical Therapist, you will have the opportunity to work with a diverse range of patients and provide high-quality care in a dynamic and supportive environment.
Job Summary:We are looking for a dedicated and compassionate Physical Therapist to join our team. The successful candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Responsibilities:- Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
- Establish a plan of care and implement individual treatment based on evaluation results and functional goals.
- Monitor and evaluate outcomes and appropriately adjust the individualized treatment plan based on the patient's response to treatment and provide status updates to physicians.
- Identify factors affecting patient's physical and psychosocial comfort and attend to these needs.
- Plan for and complete patient discharge including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
- Complete all required documentation following department policy, including patient progress notes, team conference reports, and discharge summaries as appropriate, to record patient care and progress of each patient assigned as well as daily statistics, billing, and scheduling.
- Apply insurance guidelines regarding authorization and number of visits.
- Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly.
- Perform various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Demonstrate skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist.
- Demonstrate knowledge of indications and contraindications of modalities and exercise.
- Participate in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care.
- Manage assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs.
- Pursue professional growth through participation in continuing education courses, department in-services, and reading professional literature.
- Orients new staff to clinical and department policies as requested.
- Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.
- Physical Therapist license issued by the state in which the team member practices (Wisconsin or Illinois).
- Bachelor's Degree in Physical Therapy.
