Key Responsibilities

  • Advises the director of operations on employment decisions regarding staff, including hiring, termination, promotions, and status changes.
  • Ensures the overall quality of patient care and the clinical performance of nursing staff and aides.
  • Coordinates clinical operations for the care center.
  • Evaluates service requests and determines patient eligibility for home care services, collaborating with intake for patient referrals and admissions.
  • Manages scheduling operations.
  • Oversees payor verification and precertification processes.
  • Reviews documentation from staff and addresses any issues related to missing, incomplete, or late documentation.
  • Assists clinicians in establishing immediate and long-term patient goals, prioritizing needs, and developing the Plan of Care (POC).
  • Regularly assesses patient needs and collaborates with clinicians to revise the plan of care as necessary.
  • May provide direct patient care during emergencies on an infrequent basis.
  • Participates in the on-call rotation.
  • Facilitates staff training to ensure quality care, compliance, and fiscal responsibility.
  • Engages in the investigation and resolution of patient, family, and physician concerns.

Qualifications

  • Minimum of one year of clinical RN experience in home health or hospice settings.
  • At least one year of RN management experience.
  • Current RN license specific to the state(s) of employment.
  • Current CPR certification.
