Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at Lancaster General Health. As a key member of our healthcare team, you will provide high-quality patient care and work closely with our providers to deliver exceptional patient outcomes.
Responsibilities
Provide clinical advice by phone or electronic correspondence based on clinical judgments and/or triage protocols in accordance with regulatory requirements and standards of practice.
Consult with providers as necessary to ensure seamless patient care.
Provide patient-specific education on preventive care, chronic disease management, acute diagnoses, medications, and other areas pertinent to patient outcomes.
Prepare patients for visits by using appropriate rooming standards tools.
Administer injections and medications as prescribed by providers and in accordance with department protocols, regulatory requirements, and standards of practice.
Prepare and assist providers with various procedures, minor surgeries, and other exams as designated by providers.
Perform Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
Collect blood or other laboratory specimens and prepare them for further testing.
Authorize prescription refills as instructed by providers.
Assist with test tracking and monitoring overdue results.
Remove sutures and change dressings on wounds as necessary.
Ensure exam rooms are adequately supplied and cleaned after each patient visit.
Maintain and clean medical instruments and equipment after each procedure.
Assist providers and patients in reviewing and revising care plans and assess patient progress.
Follow up with patients between visits as necessary.
Assist with proactive outreach for patients requiring preventive and chronic disease services.
Perform care coordination activities among specialists and transitions of care to obtain all necessary patient information.
Assist in coordination of referrals, education, and community resources.
Actively participate in performance improvement initiatives to improve overall patient experience and quality of care.
Participate in daily huddles with providers.
Serve as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors, and colleagues.
Communicate with all patients, visitors, and colleagues in a professional manner while keeping customer service a top priority.
Handle all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures.
Requirements
Completion of a LPN or a RN educational program.
Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing.
AHA CPR/BLS certification required within 30 days of hire.
Occasional regional travel required. Ability to travel to multiple sites required.
Preferred Qualifications
One (1) to Two (2) years of nursing experience in a primary or specialty care physician's office.
Prior triage, emergency room, cardiac care, intensive care, or other related specialty nursing experience.
