About the Role:
We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care.
Responsibilities:
- Perform patient start of care comprehensive assessment, evaluation, development, and implementation of plan of care.
- Complete OASIS assessments timely and accurately.
- Provide skilled nursing care to patients in accordance with the plan of care and agency policies.
- Help achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members, and community resources.
- Initiate appropriate referrals when indicated.
- Promote and maintain patient health through teaching and appropriate rehabilitative measures.
- Assist patients in learning appropriate self-care techniques.
Qualifications:
- Current RN license, specific to the state(s) you are assigned to work.
- One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
