We are seeking a dedicated and compassionate Speech Therapist to join our team of skilled home healthcare providers. The ideal candidate will have a strong commitment to providing quality care to our patients in their homes.
Responsibilities- Evaluate the client's level of functioning and hearing, and recommend mechanisms to enhance the client's hearing ability.
- Provide initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Establish a Speech Therapy treatment plan in consultation with the physician and revise it with the physician's approval as necessary.
- Assist in the development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.
- Communicate with other team members and instruct them in speech pathology techniques as needed.
- Teach, supervise, and counsel the family and client in the total Speech Therapy program and other related problems of the client at home.
- Record all evaluation data, treatments, and client's response to therapeutic intervention on a timely basis.
- Supervise the Home Health Aide every 14 days when Speech Therapy is the only professional discipline involved in the client's care.
- Record and report to the physician the client's reaction to treatment and/or any changes in the client's condition.
- Participate in the development of periodic revision of the physician's plan of treatment and keep them informed of the patient's status.
- Graduate of an accredited Speech Therapy program or equivalent education.
- Licensed in the State of Florida as a Speech Therapist.
- Two years of clinical experience as a Speech Therapist.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
