HEART OF GOLD SUPPORT SERVICES is seeking a dedicated and compassionate Caregiver to join our team. As a Caregiver, you will play a vital role in providing support and assistance to individuals with special needs in their homes and communities.
Key Responsibilities- Provide one-on-one support and assistance to consumers in their homes and communities
- Help consumers achieve their Individualized Plan of Care (IPP) objectives
- Maintain accurate documentation of consumer progress
- Ensure confidentiality and maintain consumer information
- Take case notes and participate in assessment, planning, and evaluation processes
- Provide transportation for consumers as needed
- Coach consumers based on approved curriculum
- Communicate with consumers' probation officers and provide updates on program progress
- Respond to law enforcement issues and provide court representation as needed
- Teach and instruct consumers in all areas of curriculum
- Excellent communication and phone etiquette skills
- Strong organizational and time management abilities
- Proficiency in calendar management and scheduling software
- Experience in team management or supervision is a plus
- Knowledge of medical office management is preferred but not required
