HopeHealth is in search of a skilled hospice aide to provide care for patients in their residences and in long-term care facilities across various regions.
The hospice aide is responsible for delivering personal care and performing light housekeeping tasks as outlined in the care plan. This unique role allows you to engage meaningfully with patients, offering them the compassionate support they require. At HopeHealth, hospice aides are valued members of the interdisciplinary hospice team.
The responsibilities of a hospice aide are determined and assigned by a licensed nurse to address the needs of the patient and their family.
This is a full-time role with standard hours and includes eligibility for benefits.
Key Responsibilities of a Hospice Certified Nursing Assistant (CNA):
- Deliver personal care and assist with light housekeeping duties
- Monitor, report, and document patient conditions and the care provided
- Offer compassionate support to patients, families, and caregivers through ongoing communication
- Adhere to basic infection control protocols
- Recognize and report changes in the patient's condition to the RN Case Manager
- Identify emergencies and apply appropriate emergency procedures
- Maintain confidentiality of patient and family information
Qualifications for the Certified Nursing Assistant (CNA) Role:
- Certification as a Nurse Aide
- High School Diploma or equivalent
- 1-3 years of experience in home care and long-term care environments is required.
