Home Health Care Registered Nurse Job Description

BAYADA Home Health Care is seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality home health care services to our clients in the Norristown, Pennsylvania area.

Job Responsibilities:
  • Develop and implement personalized care plans for clients, ensuring their unique needs are met.
  • Conduct thorough assessments of clients' conditions, identifying areas for improvement and developing strategies for care.
  • Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
  • Monitor clients' progress, reporting any changes or concerns to the Clinical or Client Services Manager.
  • Collaborate with clients, families, and healthcare teams to ensure seamless care transitions and optimal outcomes.
Requirements:
  • A current license as a Registered Nurse in Pennsylvania.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience is strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills and a patient-centered approach.
  • Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
