Key Responsibilities:

  • Conduct thorough initial and ongoing evaluations of residents' medical and psychosocial needs.
  • Diagnose and treat a wide range of acute and chronic medical conditions common in older adults.
  • Develop and implement individualized care plans in collaboration with nursing staff, therapists, and other healthcare professionals.

Requirements:

  • Board-certified or board-eligible in Geriatric Medicine or Internal Medicine.
  • Valid medical license in the state of NH (or working towards).
  • Minimum of 2 years of experience working in a geriatric setting (e.g., nursing home, long-term care facility).
  • Excellent communication, collaboration, and interpersonal skills.
  • Compassionate and patient demeanor with older adults.
  • Strong commitment to resident-centered care and quality improvement.
  • Proficient with EPIC EMR.
