Key Responsibilities:

* Partner with adults with disabilities to achieve their unique goals, tailoring community-based activities to their interests and aspirations.
* Develop, review, and evaluate individual program plans, tracking progress and celebrating milestones along the journey.
* Build strong relationships with participants, families, and community partners, fostering a supportive network for success.

Requirements:
