About the Role

We are seeking a highly skilled MDS Coordinator to join our team at Wellbrooke of Westfield. As a key member of our care team, you will be responsible for assessing the needs of our residents and creating personalized care plans to ensure their comfort and well-being.

Key Responsibilities
  • Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs and create a unique care plan based on your findings.
  • Promote cross-functional communication to ensure care plans are followed.
  • Ensure appropriate reimbursement for resident care services.
  • Stay up-to-date with current federal, state, and local standards, guidelines, and regulations.
Requirements
  • Must have and maintain a current, valid state RN or LPN license.
  • 2+ years of MDS (resident assessment) experience.
  • Prior experience in Long-Term Care is required.
