Care Coordinator

As a Care Coordinator at Trilogy, you will be responsible for assessing residents' needs and creating personalized care plans. Our team is dedicated to providing exceptional care and support to our residents, and we are looking for a skilled and compassionate individual to join our team.
Key Responsibilities:
Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs
Promote cross-functional communication to ensure care plans are followed
Ensure appropriate reimbursement for resident care services
Stay up-to-date with current federal, state, and local standards, guidelines, and regulations
Requirements:
Current, valid state RN or LPN license
2+ years of MDS (resident assessment) experience
Prior experience in Long-Term Care is required
