Salary: $60,000 - $70,000 per year

Job Summary

The Patient Care Coordinator will be responsible for meeting with new clients, orienting them to our services, and staying with them until discharge. You will also collect information from clients at regular intervals to track their outcomes and perform care coordination duties as needed.

Key Responsibilities

  • Meet with new clients and orient them to our services
  • Familiarize clients with community resources available for educational, cultural, recreational, and social activities
  • Perform care coordination duties, including assisting clients with accessing physical health care, social services, housing resources, and employment resources
  • Drive clients to appointments, take them to the grocery store, pharmacy, etc., meet them in public settings or in their home

Required Skills and Qualifications

  • Bachelor's degree in a human service field or equivalent work experience
  • Strong oral and written communication skills using the English language
  • Basic personal computer skills which enable the entry, retrieval, and use of electronic data
  • Experience with Microsoft Office software
  • A valid driver's license and automobile liability insurance maintained in accordance with Pawnee policy and procedure
