We are looking for a dedicated and experienced Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to improve patient mobility and function
  • Collaborate with nursing and other healthcare professionals to ensure seamless care coordination
  • Continuously assess and revise patient care plans to ensure optimal outcomes
Requirements
  • Active and current license to practice as a Physical Therapist in the state of New Jersey
  • Previous home healthcare experience preferred, but not required
  • Ability to travel to patient homes as assigned
  • Strong communication and interpersonal skills
