Home Health Physical Therapist

BAYADA Home Health Care is seeking a compassionate and skilled Home Health Physical Therapist to join our team in Mantua, New Jersey.
About the RoleAs a Home Health Physical Therapist, you will play a vital role in providing high-quality patient care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Conduct thorough assessments of patients' physical abilities and develop personalized care plans to improve their mobility, strength, and overall quality of life.
- Perform diagnostic tests and measurements to identify areas of improvement and track progress.
- Develop and implement manual therapeutic exercises, gait training, balance, and other interventions to help patients achieve their goals.
- Continuously assess and revise care plans to ensure optimal patient outcomes.
- Active and current license to practice as a professional Physical Therapist (PT) in the state of New Jersey.
- Previous home healthcare experience is welcomed, but not required. We will provide comprehensive training to ensure your success.
- Ability to travel to cases as assigned.
- A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
