At Option Care Enterprises, Inc., we are committed to delivering exceptional patient care through our innovative solutions. As a Clinical Transition Specialist, you will play a vital role in ensuring seamless transitions for patients from hospital to home or alternative site infusion care.

Key Responsibilities:
  • Establish and maintain strong relationships with referral sources, including hospitals and medical practices, to increase sales and patient starts.
  • Conduct assessments of patients to ensure they are viable candidates for alternative site care and provide education to patients, their families, and facility staff about our services.
  • Collaborate with internal and external partners to ensure quality of service and implementation of effective treatment plans.
  • Proactively identify operational issues and recommend strategies to resolve and improve processes.
  • Communicate effectively with agency staff, medical teams, patients, and families throughout the discharge process.

Requirements:
  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice and at least 2 years of experience in the healthcare industry.
  • Basic PC skills, including internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
  • Ability to plan, organize, and make presentations.
