RN Visiting Home Health Hospice Nurse

As a skilled and compassionate Registered Nurse, you will provide comprehensive care to patients in their homes, working collaboratively with an interdisciplinary team to deliver high-quality care in home health, palliative care, and hospice programs. Your expertise will be essential in assessing patient needs, developing personalized care plans, and educating patients and their families on health management and symptom control.
Key Responsibilities:- Collaborate with healthcare professionals and patients to develop and implement effective care plans, ensuring seamless transitions and optimal patient outcomes.
- Provide skilled nursing interventions, including pain and symptom management, and initiate preventive and rehabilitative procedures as needed.
- Direct and supervise the care of LPNs and HHAs, ensuring high-quality patient care and adherence to KFHPW standards.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and accurate documentation and care coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
- Minimum two years of experience as a Registered Nurse in medical-surgical, home health, or hospice settings.
- Associates degree in Nursing from an accredited program.
- Registered Nurse License (Washington) required at hire or Compact License: Registered Nurse required at hire.
- Basic Life Support required at hire.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Experience with home care systems, such as Horizon.
- Bachelor's degree in Nursing or a related healthcare field.
