Responsibilities:

  • Provide skilled nursing care and services to patients in their homes
  • Work collaboratively with an interdisciplinary team to develop and implement individualized care plans
  • Assess patient needs and communicate with other healthcare providers to ensure seamless care
  • Collect and document relevant data throughout the duration of care
  • Develop and initiate plans of care based on patient assessments
  • Communicate and collaborate with medical doctors to establish plans of care
  • Evaluate patient response to care through timely assessment, communication, and observation
  • Modify plans of care through assessment based on patient and care systems response

Requirements:

  • Minimum two (2) years of experience as a Registered Nurse in medical-surgical, home health, or hospice
  • Associates degree in Nursing from an accredited program
  • License, Certification, Registration
  • Demonstrated experience providing care planning within a community setting (continuum of care)
  • Basic laptop skills, including MS Office Suite
  • Horizon for home care system experience
  • Bachelors degree in Nursing or a related healthcare field
