About the Role

As a Private Duty Nurse LVN, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our care team to develop and implement personalized care plans that meet their unique needs.

Responsibilities

  • Implement physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Health promotion, teaching, and training of family members
  • Accept and write verbal orders from physicians as required
  • Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition

Qualifications

  • Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation
