We are seeking a skilled RN Evaluation Coordinator to join our multidisciplinary team at the Samsky Advanced Heart Failure Clinic. As a key member of our team, you will play a pivotal role in assisting patients through the evaluation process for heart transplant and LVAD candidacy.

Job Purpose:

Participate in the development, implementation, and maintenance of the LVAD program and the heart transplant program; to coordinate and manage the patient care of this high-risk patient population throughout the continuum.

Responsibilities:
  • Coordinate and manage patient care for high-risk patients throughout the continuum
  • Participate in the development and implementation of the LVAD program and the heart transplant program
  • Collaborate with multidisciplinary professionals, including cardiothoracic surgeons, pharmacists, registered nurses, social workers, and dietitians
  • Provide compassionate patient communication and education
  • Manage time effectively, prioritize tasks, and use independent judgment
Qualifications:
  • Graduate from a Registered Nurse Program
  • Minimum 2 years of professional clinical experience
  • Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License
  • BLS and ACLS certification required
  • Cardiovascular focus experience preferred
  • Leadership qualities, including time management, verbal and written communication skills, listening skills, decision making, priority setting, and work organization skills
