Position Summary

Are you passionate about making a meaningful impact in the lives of individuals with long-term health care needs? Molina Healthcare is seeking a dedicated professional to fill the role of an IRIS Consultant. This position offers the flexibility of remote work while providing essential support to participants enrolled in the Wisconsin IRIS Program.

As an IRIS Consultant, you will engage with participants to help them maximize their experience within the IRIS program, a Medicaid long-term care option designed for older adults and individuals with disabilities. Your role will involve understanding the unique long-term care objectives of each participant and collaborating with them to create innovative strategies to achieve these goals.

Key Responsibilities

  • Conduct a minimum of four in-person meetings annually with each IRIS participant, including one required home visit.
  • Provide comprehensive program orientation to new participants, ensuring they understand their rights and responsibilities.
  • Explore various aspects of participants' lives, including their goals, community connections, and support systems.
  • Assist participants in defining personal outcomes and ensure ongoing progress while adhering to their IRIS budget.
  • Document all planning and orientation activities promptly within the IRIS data system.
  • Research and share community resources that align with participants' individual outcomes.
  • Maintain clear communication regarding budget management and assist participants with financial documentation.
  • Handle sensitive communications with confidentiality, including programmatic changes and budget adjustments.
  • Collaborate with fellow consultants to ensure effective implementation of participant plans.
  • Participate in regular team meetings and check-ins to discuss progress and strategies.

Qualifications

Education

A Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a related field is required. Alternatively, four years of relevant experience may be considered in lieu of a degree.

Experience

  • Minimum of one year of direct experience in delivering social services to individuals with disabilities or older adults.
  • Ability to work independently and demonstrate self-motivation.
  • Familiarity with Long-Term Care programs and principles of self-determination.
  • Strong problem-solving and critical thinking skills.
  • Excellent time management and organizational abilities.
  • Proficient in building and maintaining professional relationships.
  • Comfortable working with diverse populations and adapting to various situations.
  • Resourceful with knowledge of community resources and proactive in addressing participant needs.
  • Competent in using various technology platforms including Outlook, Teams, and online databases.

Licensure

A valid driver's license with a good driving record is required, along with reliable transportation.

Compensation

Pay Range: $35.17 per hour. Actual compensation may vary based on geographic location, work experience, education, and skill level.
