Job Summary

We are seeking a skilled Home Health Liaison to join our team at Advantage Home Health and Hospice. This role will be based out of our office in Indiana, PA, and will involve building relationships with referral sources and educating them on our Home Health services.

Key Responsibilities:

  • Develop and present in-services to referral sources and follow up with them to ensure their needs are being met.
  • Educate referral sources on the components of Home Health services and provide ongoing support.
  • Serve as a liaison between Home Health and physicians, hospitals, long-term care facilities, and other referral sources.
  • Communicate regularly with referral sources to ensure that their expectations and the needs of our patients are being met.
  • Identify and resolve any issues or problems that referral sources may be experiencing with our quality of care.
  • Monitor primary relationships for consistency and quality.
  • Submit written documentation of all contacts, referrals, and contracts in a timely manner.
  • Assist in the development of new programs to meet the needs of our community.

About You:

  • You have 3-5 years of experience in Hospice or Home Health.
  • You have a proven track record of success in reaching and exceeding quotas.
  • You possess excellent organizational, communication, and interpersonal skills.
  • You are a strategic thinker who can help evolve our marketing calendars and processes.
  • You are a self-starter with a strong sense of urgency and initiative.
  • You have the ability to multitask in a fast-paced environment.
  • You work well with multiple teams and partners, and have solid computer skills (Excel, Word, PowerPoint, etc.).
  • You are a positive collaborator who balances taking initiative and direction as needed.
  • You have the ability to work collaboratively with multiple cross-functional teams: sales, marketing, operations, etc.
