Responsibilities- Clinical Competence: Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Provide direct and indirect supervision of home health aides and licensed practical vocational nurses, ensuring compliance with state and federal regulations.
- Documentation and Care Delivery: Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements. Accurately and timely complete comprehensive assessments, including medication reconciliation, and document patient visits per policy.
- Quality: Ensure patients meet home health eligibility and medical necessity guidelines, accurately determine primary focus of care, and develop plans of care within state-specific guidelines. Communicate relevant information effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
- Professionalism and Teamwork: Demonstrate a positive attitude, willingness to learn, and punctuality for meetings. Participate in on-call rotation, weekend rotation, and survey readiness activities, adhering to the agency's utilization management model.
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
