Job Summary

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health RN, you will have the flexibility to create your own schedule and work at your own pace, providing exceptional patient care in the comfort of their own homes.

Key Responsibilities
  • Provide high-quality, patient-centered care to individuals with various medical conditions, including illnesses, injuries, and chronic diseases.
  • Work collaboratively with a multidisciplinary team, including physicians, LPNs, CNAs, and aides, to develop and implement individualized care plans.
  • Conduct thorough assessments, document progress, and communicate effectively with patients and their families.
  • Educate patients and their families on disease management, medication, therapy, diet, and exercise.
  • Ensure seamless transitions of care and coordinate discharge from services.
Requirements
  • Graduate of an accredited nursing program and active RN license in IL.
  • Minimum of 2 years of nursing experience, preferably in home healthcare.
  • CPR certification and BLS certification.
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion.
