About the Role

VNS Health is seeking a highly skilled Nurse Specialist to join our team in a nursing job that focuses on wound care. As a key member of our healthcare team, you will be responsible for providing expert clinical care and developing programs to improve patient outcomes.

Key Responsibilities
  • Develop and implement evidence-based care practices to improve patient outcomes and reduce hospitalizations.
  • Collaborate with staff to improve clinical outcomes and provide expert consultations, staff development plans, and continuous quality improvement strategies.
  • Evaluate clinical outcomes data and analyze practice patterns to promote organizational responses that meet the needs of patients with complex and chronic illnesses.
  • Develop policies, standards, and protocols for the application of evidence-based practice and evaluate their impact on care management outcomes.
  • Collaborate with program leadership to identify, develop, implement, and evaluate practice improvement plans.
  • Educate, mentor, coach, and develop clinical staff to improve quality of care and reduce transition problems from hospital to home.
  • Provide clinical care in accordance with VNS Health policies, practices, procedures, and Standards of Professional Practice.
  • Participate in special projects and perform other duties as assigned.
Requirements
  • License and current registration to practice as a Registered Professional Nurse in the State of New York within 6 months of hire required.
  • Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational needs.
  • Clinical certification in a specific specialty, such as diabetes, oncology, wound/ostomy/continence, etc. preferred.
  • Bachelor's Degree in Nursing required.
  • Master's Degree in Nursing preferred.
  • Minimum of five years clinical experience required.
  • Experience developing and delivering disease-specific educational programs preferred.
  • Effective oral/written communication and interpersonal skills required.
  • Proficient with personal computers, including Microsoft Office Word and Excel preferred.
