Job Summary:

The Hospice Nursing Assistant will provide personal care and support to patients in their homes, hospitals, and other care settings. This includes assisting with daily living activities, providing emotional support, and collaborating with the healthcare team to ensure the best possible outcomes for our patients.

Responsibilities:
  • Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming.
  • Assist patients with mobility and transfer needs, ensuring their safety and comfort.
  • Prepare nutritional snacks and meals as needed, following dietary guidelines and patient preferences.
  • Assist with passive and active range of motion exercises, as directed by the nurse.
  • Perform simple wound care, skin care, and bowel care protocols, under the supervision of the nurse.
  • Recognize and report any changes in the patient's mental or physical status to the nurse.
  • Record patient activities and observations in the clinical record, as required.
  • Participate in patient care conferences and team meetings to ensure seamless care coordination.
  • Attend in-services and education sessions to maintain competency and stay up-to-date on best practices.
Requirements:
  • Must be a state-tested nursing assistant in good standing.
  • Must be able to read and write English proficiently.
  • Must be able to lift, push, and pull patients and equipment, as needed.
  • Must be able to work in a fast-paced environment and prioritize tasks effectively.
