We are seeking a skilled Mobile MDS Coordinator to join our team at Saber Healthcare Group. As a Mobile MDS Coordinator, you will play a critical role in ensuring the highest quality of care for our residents.
Job Summary:The primary purpose of this position is to complete the Resident Assessment Instrument (RAI) process in accordance with state and federal laws, as well as the policies and goals of our community. You will be responsible for assessing the needs of our residents, developing and implementing care plans, and ensuring that our residents receive the highest level of care.
Responsibilities:- Assess the needs of residents and develop individualized care plans
- Complete the RAI process in accordance with state and federal laws
- Collaborate with healthcare professionals to ensure the highest quality of care
- Monitor and evaluate the effectiveness of care plans
- Communicate effectively with residents, families, and healthcare professionals
- Active, unencumbered Registered Nurse license in the state of practice
- One year of experience in working with the RAI process
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
