As a Case Manager with TAG MedStaffing, you will play a vital role in delivering high-quality case management services to clients in need. In this position, you will conduct comprehensive assessments, develop tailored service plans, and engage in fieldwork to support clients in achieving their goals and maintaining independent living.
Key Responsibilities:
- Client Support: Provide personalized case management services to clients, ensuring their needs are met and they receive the support they require.
- Assessment & Planning: Conduct thorough assessments and develop service plans with achievable goals, adjusting as necessary to ensure client success.
- Client Interaction: Perform regular home and office visits to monitor client progress and provide ongoing support.
- Health & Wellness: Assist clients in accessing healthcare services and promoting daily living activities to improve overall well-being.
- Crisis Management: Provide effective crisis intervention to address immediate client needs and ensure their safety.
- Housing Assistance: Offer guidance on tenant rights, support housing transitions, and ensure financial compliance to help clients maintain stable living arrangements.
Qualifications:
- Education: Bachelor's Degree in Social Work or Public Administration preferred (relevant experience considered).
- Experience: 2+ years of experience in counseling, particularly with mental health and substance abuse populations.
- Skills: Strong organizational and communication skills with a compassionate approach.
- Technical Proficiency: Basic computer skills, including Microsoft Excel and Word.
- Language: Bilingual in Spanish is a plus
