Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to deliver exceptional outcomes.

Key Responsibilities:
  • Conduct thorough assessments and develop personalized care plans for patients with various medical conditions
  • Provide direct patient care, including medication management, wound care, and education on disease management and self-care
  • Collaborate with physicians, LPNs, CNAs, and other healthcare professionals to ensure seamless care coordination
  • Document patient progress and communicate effectively with the care team
  • Ensure patient safety and well-being by identifying and mitigating potential risks
Requirements:
  • Active Registered Nurse (RN) License in Colorado
  • Graduate of an accredited school or program of nursing
  • At least 1200 hours of experience as an RN in healthcare
  • Healthcare CPR/BLS Certification
  • Home health care experience preferred
