Responsibilities- Assess and Plan Patient Care: Utilize the nursing process to assess patient needs, develop individualized care plans, and implement evidence-based interventions to promote optimal patient outcomes.
- Provide Direct Patient Care: Perform a range of nursing tasks, including administering medications, wound care, and vital sign monitoring, to ensure the physical and emotional well-being of our patients.
- Collaborate with Healthcare Team: Work closely with our interdisciplinary team, including physicians, therapists, and other healthcare professionals, to coordinate patient care and achieve shared goals.
- Document Patient Information: Accurately and thoroughly document patient information, including medical histories, treatment plans, and progress notes, to ensure continuity of care and facilitate communication among healthcare team members.
- Current LVN License: Possess a current and active Licensed Vocational Nurse (LVN) license in the state where you will be practicing.
- Health Certificate: Hold a current Health Certificate, as required by our facilities.
- PPD or Chest X-Ray: Have a current PPD or Chest X-Ray, as required by our facilities.
- BLS Certification: Possess a current BLS (Basic Life Support) certification.
- Experience: One year of prior experience as an LVN is preferred, but not required.
