Key Responsibilities:

  • Provide high-quality, patient-centered care to patients in their homes

  • Prescribe medications and order diagnostic tests as needed
  • Communicate effectively with patients, families, and healthcare teams

Requirements:

  • Current state RN license (if Nurse Practitioner)
  • Current NP or PA license
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
  • Proven knowledge of long-term care, community resources, and cost-effective alternatives
