Key Responsibilities:

  • Provide care management to a caseload of 100 members, including assessing for health care needs, referring to health plan providers, and providing education on benefits/expanded benefits.
  • Document in an electronic health record and maintain accurate and up-to-date records.
  • Develop and implement care plans for each member, taking into account their unique needs and circumstances.
  • Collaborate with healthcare providers, community resources, and other stakeholders to ensure seamless care coordination.
  • Provide community resources to address social determinants of health needs, such as housing, employment, and education.

Requirements:

  • Strong understanding of healthcare systems and policies.
  • Excellent communication and interpersonal skills.
