Job Summary

We are seeking a highly skilled and compassionate Care Partner to join our team at Twin Fountains Home. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and promoting a positive patient experience.

Responsibilities
  • Provide support care to patients under the direction of the Registered Nurse
  • Assist with daily living activities and comfort measures
  • Perform basic patient care functions, including phlebotomy, simple treatments, and dressing changes
  • Transport unit supplies, patient care supplies, medications, and equipment as assigned
  • Document patient care in the Electronic Medical Record (EMR)
  • Communicate patient care concerns, problems, and emergencies to the RN or Charge Nurse in a timely manner
Requirements
  • High school diploma or equivalent required; graduation from a State-approved program for Certified Nursing Assistants or Patient Care Tech Program preferred
  • Ability to read, write, and speak the English language
  • Basic computer skills required
  • Ability to collaborate with other healthcare providers in the provision of patient care
Working Conditions

This is a fast-paced environment that requires multi-tasking and effective communication with the Registered Nurse and ancillary team. The Care Partner must be able to work in a team environment and take direction from a variety of care providers.
