About the Role

We are seeking an experienced Occupational Therapist (OT) to join our team at Aequor Technologies. As an OT, you will play a vital role in helping patients regain or improve their independence in daily activities within a Skilled Nursing Facility (SNF) setting.

Key Responsibilities

Assess patients' physical and mental abilities to determine rehabilitation needs, focusing on activities of daily living (ADLs) and functional mobility. Develop and implement individualized treatment plans to help patients achieve optimal outcomes. Provide therapeutic interventions to enhance fine motor skills, coordination, strength, and mobility, using a variety of techniques and equipment. Collaborate with a multidisciplinary healthcare team to ensure comprehensive patient care. Monitor and document patient progress, adjusting treatment plans as necessary. Assist in the discharge planning process, providing recommendations for continued therapy and support services. Maintain a clean, organized, and safe therapy environment, adhering to infection control and safety protocols.

Requirements

Strong understanding of occupational therapy principles, especially in the context of a Skilled Nursing Facility (SNF) environment. Excellent communication and interpersonal skills for interacting with patients, families, and a multidisciplinary team. Proficiency in creating and implementing individualized treatment plans based on patient needs. Active [STATE] Occupational Therapist (OT) License in good standing. A minimum of 2 years of experience as an Occupational Therapist or in a similar role. Certification in Basic Life Support (BLS). Certification in Advanced Cardiovascular Life Support (ACLS). Experience with EPIC or similar charting systems.
