Job DescriptionThis role assumes primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.
- Determine patient needs and initiate intervention by other members of the interdisciplinary team.
- Communicate with the attending physician, interdisciplinary team, and other agencies for optimal care coordination.
- Provide regular visits to reassess patient needs and update the plan of care.
- Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely.
To be successful, you should have:
- A graduate degree from an accredited registered nursing program.
- A valid registered nurse license in the state of practice.
- Current CPR and First Aid Certification.
- Knowledge of quality assessment and assurance procedures.
- A valid driver's license and proof of insurance.
The estimated salary for this role is $85,000 - $110,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including health insurance, dental insurance, vision insurance, life insurance, disability insurance, and a 401(k) retirement plan.
