We are seeking a highly qualified and compassionate PRN RN Home Health professional to join our team. This exciting opportunity allows you to make a meaningful difference in the lives of our patients and their families while enjoying the flexibility and autonomy that comes with working as an independent contractor.

About the Role

As a PRN RN Home Health nurse, you will provide coordinated skilled nursing care to patients of all ages in their homes. Your primary responsibilities will include:

  • Conducting thorough assessments of patients' physical, psychosocial, and environmental needs
  • Developing and implementing personalized care plans that meet the unique needs of each patient
  • Providing direct patient care, including medication administration, wound care, and education on self-care techniques
  • Collaborating with other members of the healthcare team, including physicians, therapists, and social workers, to ensure seamless coordination of care
Requirements and Qualifications

To be successful in this role, you must possess:

  • A graduate degree from an accredited school of professional nursing
  • At least one year of experience in home health, public health, or acute hospital nursing
  • Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS (Strongly preferred)
  • Reliable transportation and proof of valid automobile liability insurance
  • A valid driver's license
