Job SummaryWe are seeking a highly skilled Client Care Coordinator to join our team and play a pivotal role in building out best program practices, executing deliverables, and contributing to the continued development of our GoGoCare program. As a key member of our care team, you will be responsible for enrolling and onboarding new clients, providing telephonic prescription management and care navigation services, and collaborating with various team members to ensure seamless service delivery.
Key Responsibilities- Serve as the primary point of contact for GoGoCare services, ensuring exceptional customer experience and satisfaction.
- Enroll new clients into GoGoCare, conducting thorough assessments to understand their needs and preferences.
- Provide telephonic prescription management services, including working with clients' providers and pharmacists to obtain timely medication refills, conducting medication assessments, and educating clients on medications and medication side effects.
- Assess client needs and provide general care navigation and care coordination services, documenting client interactions and monitoring client outcomes.
- Assist in the development of program processes and best practices, collaborating with various team members, including social workers, engineering, and contact center representatives.
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) with a valid and in-good-standing license, preferably in a compact state or California.
- A minimum of 3-5 years of experience working with geriatric populations, with management experience being a plus.
- Strong competence in medication management and care navigation, with experience conducting clinical telephone assessments.
- Excellent verbal and written communication skills, with strong customer service skills and experience conducting business, care coordination, and education, especially over the phone.
- Strong computer literacy, including experience with Electronic Health Records (EHR) and Google Workspace, with familiarity with Slack being a plus.
- Bonus: Spanish speaking is a plus.
