Occupational Therapist Job Description

Care IV Home Health is a locally owned and operated home health agency in Fayetteville, AR, seeking a part-time or PRN Occupational Therapist to join our team.

Job Summary

We are looking for a skilled Occupational Therapist to provide high-quality care to our patients in the comfort of their homes and assisted living facilities.

Responsibilities
  • Visit patients in their homes and assisted living facilities to provide occupational therapy services
  • Create occupational therapy care plans and document patient progress
  • Update the Registered Nurse (RN) Director on patient status and admit/discharge patients as needed
  • Perform other OT duties as assigned
Requirements
  • Occupational Therapy License actively licensed or registered in the state of Arkansas
  • Previous acute OT experience
  • A minimum of three professional references on the application or upon initial contact
