Occupational Therapist Job Description

Care IV Home Health is a locally owned and operated home health agency in Fayetteville, AR, seeking a part-time or PRN Occupational Therapist to join our esteemed team.

Key Responsibilities:
  • Visit patients in the comfort of their homes and assisted living facilities
  • Create occupational therapy care plans, perform OT, document/chart patient progress, and update the Registered Nurse (RN) Director on patient status
  • Admit and discharge patients, as well as perform other OT duties as assigned
Requirements:
  • Occupational Therapy License actively licensed or registered in the state of Arkansas
  • Previous acute OT experience
  • A minimum of three professional references on the application or upon initial contact
