Primary Care Physician Assistant

We are seeking an experienced APRN/NP to join our Adult Medicine clinic team. The successful candidate will provide comprehensive primary health care, including health promotion and disease prevention activities for patients.
About the Role
The incumbent will work in collaboration with department physicians, referring patients to consultants as necessary. They will possess a deep knowledge of nursing theory and practice or physician assistant theory and practice, coupled with a record of providing high-quality, comprehensive, professional clinical care to patients.
Responsibilities
* Provide specialized care to adult patients
* Collaborate with department physicians and refer patients to consultants as necessary
* Conduct medical histories, assess medical conditions, and interpret findings
* Communicate effectively with patients, peers, and other stakeholders both orally and in writing
* Maintain timely, thorough, and accurate written clinical documentation
* Make evidence-based clinical decisions and comply with department and organizational quality measures
* Perform other duties as assigned
Requirements
* Graduate of an accredited school of nursing and completion of an approved nurse practitioner program
* Valid State of Connecticut licensure
* Eligibility to obtain DEA and state narcotic registration
* Ability to provide cultural competent care to a diverse patient population in a collaborative environment
